To understand how a wedge pillow works, it is essential to first understand the mechanics of snoring. Snoring is essentially the sound of obstructed breathing. It occurs when air cannot flow freely through the passages at the back of the nose and mouth. As you progress from light sleep into deep sleep, the muscles in the roof of your mouth (the soft palate), your tongue, and your throat naturally relax [1, 2]. For some individuals, these tissues relax to the point where they partially block the airway. When you breathe in and out, the air is forced through this narrowed passage, causing the relaxed tissues to vibrate against each other. Anatomy plays a significant role; individuals with a low, thick soft palate, enlarged tonsils or adenoids, or an elongated uvula are more prone to airway obstruction [1, 2]. Lifestyle choices also have a profound impact. The consumption of alcohol before bedtime acts as a muscle relaxant, further decreasing the natural tone of the throat muscles and increasing the likelihood of airway collapse [1, 2]. Similarly, sleep deprivation can lead to excessive muscle relaxation during sleep. Weight is another critical factor, as individuals who are overweight may have extra tissue in the back of their throats that can narrow the airway [1, 2].
Perhaps one of the most significant and easily modifiable factors is sleep position. Snoring is typically most frequent and loudest when an individual sleeps on their back [1, 2]. In the supine position, gravity pulls the relaxed tongue and soft tissues downward, directly into the back of the throat, exacerbating the obstruction and the resulting vibrations. Understanding this gravitational effect is key to recognizing why positional therapy, such as using a wedge pillow, can be so highly effective.
